Abstract:

Untreated maxillary cysts??whether odontogenic, non odontogenic, or pseudocysts??can result in severe complications that compromise both oral health and overall quality of life. These complications may include extensive bone loss with reduced structural resistance and risk of pathological fractures, secondary infections with acute episodes, pain and swelling of considerable extent, as well as damage to adjacent anatomical structures such as teeth, the nasal cavity, maxillary sinuses, and the mandibular canal with its contents. The purpose of this presentation is to assess the utility of various classical and modern materials, techniques, and technologies in developing individualized treatment protocols tailored to patient-specific characteristics, thereby ensuring successful management of these lesions. This presentation will highlight diagnostic and therapeutic advances such as cone beam computed tomography (CBCT), fractal and radiomic analysis, piezoelectric surgery, navigation-based endoscopic enucleation (NBEE), the comparison between simple enucleation and marsupialization/decompression or their combinations, as well as the adjunctive use of autologous blood concentrates, collagen membranes, and bone grafts from various sources for guided bone regeneration. The theoretical framework will be complemented by a series of clinical cases reflecting personal experience in the management of maxillary cysts.
