Abstract:

The aim of this study was to evaluate the outcome of postpartum vaginal repair after injection of PRP immediately post vaginal repair. The investigation was designed as an observational study. The investigation was undertaken at tertiary referral unit in KSA (Saudi German Hospital, Aseer). The participants comprised of 70 women asking for vaginal repair postpartum between Oct’ 2017 to Jul’ 2018. Pre-delivery evaluation of vaginal laxity widening, the distance between the vagina and anus, any varicosities, masses at the time of delivery, injection of (20 iu units vasopressin diluted in 30 ml normal saline and 20 ml lidocain 2%) in the site of median episiotomy and the perineum, the re-evaluation after delivery, during delivery of the head, median episiotomy was done. Dissection of the vaginal mucosa on both sides from the underlying tissue was done. Approximation of the perineal muscles by 2-3 stitches. Removal of excess vaginal tissue and the suturing of the vaginal edges starting from the apex till the muco-cutanous junction. Then the perineal muscles were tied. Finally, the skin was closed subcuticularly and injection of the prepared PRP in different layers. Vaginal back was inserted to ensure heamostasis and postoperative analgesia was given mainly nonsteroidal ant inflammatory rectal suppository. High risk pregnancies e.g., under anticoagulants, PET, sever vulval varicosities, huge cystocele. Healing, cosmetic appearance was evaluated objectively. Operative and postoperative complications were also assessed. Follow-up of the women was after 2-4 and 6/8 weeks. The mean age of 70 women was 31 years (19-42 years), All were evaluated for follow up in the clinic for 2-4 and 6/8 weeks. The procedure was successful in all cases with no infection, no breakdown of the wound with better cosmetic results. The procedure is both feasible and effective procedure for achieving good vaginal repair postpartum with almost no major complications. Advantages of this procedure is less blood loss, less time of completion of the procedure, more analgesics time, more cosmetic appearance later on and less dehesiscence of the wound. A prospective randomized clinical trial with longterm follow-up and larger number of cases is needed for further evaluation and compares the outcome.

He pursued his M.B.B.Ch. at Ain Shams University, in 1983 Cairo, Egypt. He completed his M.Sc (Ob. & Gyn.) Ain Shams University, 1989 Cairo, Egypt . In 2000 He has done his Diploma of Obstetrics & Women Health, Dublin, Ireland and got his Membership of The Royal College of Obstetricians & Gynaecologists,( MRCOG), London, UK. He have the honor to be the head of the first well constructed Gyn Cosmetic Unit in the Southern Area in K.S.A. He have a great experience in Pelvic and Gyn Cosmetic Surgery. He is a member of many Gyn and cosetic Surgery societies and organizations.He was a speaker in many national and international conferences last one in Dubai Nov/2008 International Gyn Cosmetic Conference. He have many National and International publications last one in Journal of Medical Research and Practice Vol 107,Issue 02, year 2018 (An Initial Study for New Surgical Procedure for Complete Utero- Vaginal Prolapse (Procedentia) to prevent later Vault Prolapse.
