A 55-year-old man with history of coronary stent graft implantation was hospitalized for the onset of angina. Intravascular ultrasound was performed on the anterior descending coronary artery (LAD). At the end of the angioplasty the tip of the catheter remained fractured inside the vessel. The following attempt to withdrawal the guide using a percutaneous loop snare technique also caused the entanglement of this last catheter inside the coronary vessel. Ultimately, the patient underwent a rescue coronary bypass surgery to remove the foreign material and reroute the damaged LAD. This case could represent a daily heart team challenge to be aware of.
