Patient Safety is a healthcare discipline which emphasizes the reporting, analysis and prevention of medical error. Adverse events in the health care occur not because health care professionals are intentionally harming patients, but rather due to the complexity of health care systems, where successful treatments and outcomes mostly depend on many factors in addition to the competence of the health care professionals.
